My fee structure, services, and fee policy are below. They are subject to change but you will always be informed in advance before a change occurs. Please expect an increase of at least 4% on the first of every year. To see other policies relating to SelfWitte, see Policies.

Session fees

  • $175.00 for a 50 minute session

I do not currently take insurance. However, I am able to provide a superbill that can be submitted for out of network benefits for some insurance providers. You will likely want to call your insurance company and ask whether they provide out-of-network benefits and if they cover services from Licensed Mental Health Counselor Associates (LMHCA) for CPT code 90837. Other things to discuss with them include if they limit the number of sessions per year, what percentage they reimburse, and if there is a deductible you must first meet.

Sliding scale fees

Sliding scales are offered and administered through Open Path Collective. There are a limited number of slots and clients who use a sliding scale session are not guaranteed future sliding scale sessions. If client financial circumstances change to allow for a higher fee, sessions may be renegotiated to a higher fee.

Fee collection

It is the policy of my practice to collect all fees at the time of booking, unless you make arrangements for payment and we both agree to such an arrangement. If for some reason a session occurs and payment has not occurred, subsequent sessions will not be held until payment is complete for the prior service.

All accounts that are not paid within thirty (30) days from the date of service shall be considered past due. If your account is past due, please be advised that I may turn past due accounts over to a collection agency or seek collection with a civil court action. By signing below, you agree that I may seek payment for your unpaid bill(s) with the assistance of a collections agency. Should this occur, I will provide the collection agency or Court with your Name, Address, Phone Number, and any other directory information, including dates of service or any other information requested by the collection agency or Court deemed necessary to collect the past due account. I will not disclose more information than necessary to collect the past due account. I will notify you of my intention to turn your account over to a collection agency or the Court by sending such notice to your last known address.

Reports

I do not write letters for emotional support animals. The fee for any other paperwork completed on your case outside the regular course of treatment is my hourly rate, billed in half hour increments. This includes writing a report, submitting a letter, filling out FMLA paperwork, and making recommendations to schools or workplaces. This work will only be done after you specifically request it and after I discuss with you how long the task will take.

If you need me to appear in court, the fee is double my therapist’s normal session fee with a minimum of 4 hours, non-refundable and paid in advance. The court appearance must be scheduled and paid in full at least two weeks in advance.

Fees for client records

The fee for any release of client records is $75. Both electronic release and printed records must be requested two weeks (14 days) in advance and be accompanied with a completed release of information form for the party receiving the records (including if they are being released to the client themselves). Records may not be released via email; instead, please indicate whether they should be released through a shared Google drive, postal mail, or fax.

Sessions, cancellations, and refunds

I agree to adhere to established session times and expect the same. We should plan to begin and finish all sessions on time.

If you’re more than 15 minutes late to a session, I’ll assume that the session is canceled, and you will be responsible for the full counseling fee. If I’m late to a session, you may choose whether to have the session as scheduled, or to postpone to a later time.

You can cancel or reschedule a session at least 24 hours in advance, without a change fee. Any cancellations within 24 hours are subject to the full session fee.

No refunds will be issued for counseling sessions after the session has occurred.

Good faith estimates

As a client, you have the right to receive a “Good Faith Estimate” in writing explaining how much your therapy services will cost. Under federal law, health care providers, including mental health providers, are required to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services. You may request a Good Faith Estimate in advance of an already scheduled psychotherapy session or at any point during your treatment.
